We understand that arriving at a doctor’s office for the first time can be nerve wracking. Filling out paperwork, learning new faces and names, and getting to know your way around the office can be an overwhelming experience. That’s why our aim for all our new patients is to remove unnecessary stress and make you feel comfortable. Download your new patient paperwork from this page and fill it out before arriving.

Personalized Treatment

Dr. Campbell works with each patient in his office on an individual basis. The questionnaire you fill out gives him a clearer picture of your physical state. And this helps him determine the best way to move forward in giving you a personalized treatment plan to get you on your path to wellness.

New Patient Forms

Please download, print off, and complete the comprehensive Symptom Survey Questionnaire. This questionnaire, plus any pertinent lab reports you may have from previous doctors, should be returned to our office at least 24 hours before your appointment. The first step is to define your health goals and what you want to change about your health status. After a thorough medical history, we will perform a Bioimpedance Analysis (an electronic test). This provides very important information about your state of health at the cellular level. Other types of testing such as blood, saliva, and other tests may be required to fully define your health challenges.
